Social Services Director

Responsibilities:

• Coordinate and oversee the facility's Social Services department.

• Complete resident assessments, care plans, and required documentation.

• Assist with admissions, discharges, and care transitions.

• Provide support, counseling, and resources to residents and their families.

• Participate in care conferences and interdisciplinary team meetings.

• Ensure compliance with state and federal regulations.

• Advocate for resident rights and quality of life.

• Build positive relationships with residents, families, hospitals, and community partners.

Qualifications:

• Long-term care or skilled nursing experience required

• Knowledge of state and federal regulations related to long-term care.

• Excellent communication, organization, and documentation skills.

• Compassionate, resident-focused approach to care.

• Ability to work independently while collaborating with an interdisciplinary team.
