About the RN MDS Coordinator Role:

As a RN Registered Nurse MDS Coordinator, you will play a critical role in the clinical documentation and assessment process. Your responsibilities will include:

• Collaborating with our interdisciplinary care team to ensure accurate and timely completion of Minimum Data Set (MDS) assessments and care plans

• Conducting resident interviews and gathering information to inform MDS assessments and care plans

• Maintaining accurate and up-to-date records of resident assessments and care plans

• Providing education and support to our care team members on MDS-related documentation and assessment processes

• Participating in quality improvement initiatives to ensure effective and efficient MDS processes

RN MDS Coordinator Requirements:

• Current licensure as a Registered Nurse in the State of New York

• Prioi experience in a long-term care setting, preferably as an MDS Coordinator

• Strong communication and interpersonal skills to effectively collaborate with the care team

• Proficiency in electronic medical records and MDS software

• Ability to maintain confidentiality and handle sensitive information
