Registered Nurse (RN) Home Care Supervisor Needed For Licensed Home Care Agency

Helping U Home Care (HUHC) is looking for an RN Home Care Nursing Supervisor to join our exceptional interdisciplinary team of home health care providers. Do you have experience working at a New York State Licensed Home Care Agency providing Supervisory and Assessment visits?  We are looking for compassionate, dedicated, and well-rounded professionals who can uphold our standards and culture.

Qualifications:

• Current and valid NYS RN License
• Minimum 1 year of home care experience doing Supervisory Visits and Assessments (Preferred)
• Ability to accurately assess patients’ health care status and psychosocial needs
• Communicate effectively, both verbally and in writing with physicians, patients and caregivers
• Experienced with Microsoft Office and HHA Exchange a plus
• Reliable means of transportation to conduct home visits in at least 2 NYC Boroughs

Responsibilities:

• Conduct in-person initial patient assessments, re-assessments, quality assurance and supervisory visits of HUHC’s field staff (paraprofessional caregivers and nurses)
• Assist with developing, implementing and revising patient’s Plan of Care
• Ensure that all HUHC caregivers adhere to the Plan of Care and Physicians Orders  
• Provide assistance to DPS and Coordination in recommending ideal caregivers for each patient, taking into account clinical and psychosocial needs of the patient
• Communicate effectively with HUHC team members to ensure each patient is receiving the highest quality home care from HUHC’s caregivers
• Additional duties as assigned

Job Type: Full-time

Salary: $80,000.00 - $100,000.00 per year

Work Location: Manhattan office but on the road at least 50% of the time
