Registered Nurse (RN)/Licensed Practical Nurse (LPN) Sub-Acute

Job Summary:

We are seeking a compassionate and experienced Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team at Shrewsbury Rehabilitation and Nursing at Southgate. As a member of our nursing team, you will be responsible for providing direct nursing care to our residents and supervising the day-to-day nursing activities performed by CNAs/GNAs and other nursing personnel. Key Responsibilities:

  • Provide direct nursing care to residents, including administering medications, treating wounds, and monitoring vital signs

  • Supervise and direct the day-to-day functions of CNAs/GNAs and other nursing personnel

  • Ensure compliance with current federal, state, and local standards, guidelines, and regulations

  • Monitor the performance of CNAs/GNAs and provide education and counseling as needed

  • Disciplinary action as necessary, and complete performance evaluations

  • Participate in the maintenance and implementation of the Center's quality assurance program for the nursing service department

  • Collaborate with other resident services to ensure that the resident's total regimen of care is maintained

Requirements:

  • Must possess a Nursing Degree from an accredited college or university, or graduate from an approved LPN/RN program

  • Must possess a current, unencumbered, active license to practice as an RN or LPN in this state

  • Must be able to read, write, speak, and understand the English language

  • Must demonstrate knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served

  • Must be able to evaluate the performance, initiate disciplinary actions, and prepare and complete performance evaluations for personnel

Preferred Qualifications:

  • 1 year of experience in a long-term care setting

  • Basic computer knowledge
