MDS Coordinator RN

Centerville Health and Rehab is seeking an experienced MDS Coordinator to oversee the MDS process and support accurate clinical documentation and reimbursement.

This role is responsible for coordinating assessments, maintaining compliance, and working closely with the interdisciplinary team to ensure accurate reimbursement and quality outcomes. The ideal candidate has strong skilled nursing experience and understands PDPM, care planning, and regulatory requirements.

Key Responsibilities
• Coordinate and complete MDS assessments in accordance with state and federal regulations
• Ensure timely and accurate MDS submissions
• Manage care planning and collaborate with the interdisciplinary team
• Monitor clinical documentation to support reimbursement accuracy
• Track Quality Measures and support improvement initiatives
• Maintain survey readiness related to MDS and documentation
• Partner with nursing leadership to improve clinical processes

Qualifications
• Active Ohio RN or LPN license preferred
• Prior MDS experience in a Skilled Nursing Facility required
• Strong knowledge of PDPM, Medicare, and reimbursement processes
• Experience with care planning and regulatory compliance
• Strong organizational and communication skills
