What You’ll Do

• Perform initial and ongoing client assessments.

• Develop and update individualized care plans.

• Provide clinical oversight and guidance to care teams.

• Communicate effectively with clients, families, and healthcare professionals.

• Ensure compliance with state regulations and agency standards.

• Routine Med Set ups for clients.

Who You Are

• Licensed RN in Colorado

• 2+ years of nursing experience (home health or hospice preferred)

• Current CPR certification

• Strong assessment and communication skills

• Valid driver’s license and reliable transportation

• Ability to work independently and manage multiple priorities
