Job Description
The RN Admissions Nurse is responsible for coordinating and completing the admission process for residents entering the skilled nursing facility. The Admissions Nurse ensures that new admissions are assessed promptly, physician orders are obtained and implemented accurately, and residents and families receive a smooth transition into the facility. The RN Admissions Nurse works collaboratively with the interdisciplinary team to ensure quality resident care, regulatory compliance, and positive customer service.
Essential Duties and ResponsibilitiesCoordinate and complete the admission process for new residents, including evenings, weekends, and hospital referrals as assigned.
Review hospital records, discharge summaries, medication lists, and physician orders prior to admission.
Perform comprehensive nursing assessments upon admission and document findings accurately.
Verify physician orders and ensure all orders are entered and implemented correctly.
Complete medication reconciliation and ensure medications are available upon admission.
Communicate with hospitals, physicians, residents, families, and facility departments regarding admission needs.
Ensure rooms, equipment, and supplies are prepared prior to resident arrival.
Initiate care plans and communicate resident needs to nursing staff and interdisciplinary team members.
Monitor residents closely during the admission transition period and report changes in condition promptly.
Ensure all admission documentation is completed accurately and within regulatory timeframes.
Educate residents and family members regarding facility services, policies, and resident rights.
Collaborate with case managers, therapy, dietary, social services, and other departments to ensure continuity of care.
Assist with insurance verification and coordination of admission information when needed.
Maintain compliance with federal, state, and facility policies and procedures.
Participate in quality improvement activities and staff meetings as assigned.
Provide direct resident care as needed.
Current and active Registered Nurse (RN) license in the state of employment.
Current CPR certification required.
Minimum of one (1) year nursing experience preferred.
Skilled nursing or long-term care experience preferred.
Knowledge of Medicare, Medicaid, and skilled nursing regulations preferred.
Strong assessment, organizational, and communication skills.
Ability to multitask and work in a fast-paced environment.
Ability to work collaboratively with interdisciplinary teams.
Proficient computer and documentation skills.
Ability to stand, walk, bend, and lift residents or equipment as necessary.
Ability to push, pull, and lift up to 50 pounds with assistance when necessary.
Ability to work extended hours or flexible schedules as needed.
Works in a skilled nursing and rehabilitation environment.
Exposure to infectious diseases, bloodborne pathogens, and bodily fluids.
May be required to work weekends, holidays, or on-call shifts.
