Independence Health and Rehabilitation Center is seeking a compassionate RN to join our highly collaborative care team, supporting patients across skilled nursing, rehab, and long-term care as they transition from hospital to home and regain independence.

What You’ll Do:

• Provide direct patient care (medications, treatments, monitoring)

• Develop care plans in partnership with an interdisciplinary team

• Assess patients and respond to changing needs

• Work closely with nurses, therapists, and providers to achieve the best outcomes

• Communicate with patients, families, and team members

• Maintain accurate documentation

What You Need:

• Active Oregon RN license

• Strong teamwork and communication skills

• Ability to thrive in a fast-paced environment

• Experience in skilled nursing, rehab, or long-term care preferred
