Admisisons Coordinator (LPN) - Licensed Practical Nurse

A nursing home near Kingsport, TN is seeking a detail-oriented and organized Admissions Coordinator (LPN) to join our team. This position serves as a key point of contact for referral sources, patients, and families while coordinating admissions and ensuring a smooth transition into our facility.

What You'll Do:

• Coordinate the admissions process from referral through admission along with the intake team

• Communicate with hospitals, physicians, case managers, and families

• Review clinical information to support admission decisions

• Collaborate with the interdisciplinary team to ensure seamless transitions

• Maintain accurate documentation and admission records

• Build and maintain strong relationships with referral partners

• Participate in marketing and census-building initiatives as needed

Qualifications:

• Current unrestricted LPN license for the state of Tennessee

• 2 years of work experience as a LPN
