Prestige Care Center of Plattsmouth is seeking a compassionate and dedicated Physical Therapist (PT) to join our rehabilitation team. Required Qualifications
Current, valid, unrestricted Physical Therapist license in the State of Nebraska.
Graduate of an accredited Physical Therapy program.
Meets all educational and licensing requirements for state practice.
Current CPR certification (preferred if applicable).
Valid driver's license.
Knowledge of skilled nursing regulations, Medicare documentation requirements, and long-term care best practices.
Strong communication, clinical reasoning, and documentation skills.
Key Responsibilities
Evaluate residents and develop individualized treatment plans designed to restore function, improve mobility, reduce pain, and maximize independence.
Provide skilled physical therapy services in accordance with physician orders, state and federal regulations, APTA standards, and facility policies.
Assess gait, balance, strength, endurance, range of motion, transfers, and functional mobility while establishing measurable treatment goals.
Implement evidence-based interventions to improve functional outcomes and reduce fall risk.
Accurately complete all clinical documentation, billing, and coding in compliance with facility and payer requirements.
Collaborate with physicians, nursing staff, occupational and speech therapists, social services, and other interdisciplinary team members to coordinate resident care.
Participate in care conferences, discharge planning, and interdisciplinary meetings.
Educate residents, families, and caregivers on therapeutic exercises, mobility techniques, fall prevention, assistive device use, and home exercise programs.
Monitor resident progress, modify treatment plans as appropriate, and ensure achievement of functional goals.
Maintain therapy equipment and treatment areas in a clean, safe, and organized manner.
Adhere to the APTA Code of Ethics, state practice regulations, and facility policies while maintaining the highest standards of professional conduct.
