About the Role:

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our night shift team! As an LPN at Green Palms Health & Rehab of St. Marys, you will play a vital role in providing high-quality care to our residents. You will work closely with our interdisciplinary team to develop and implement individualized care plans, monitor resident conditions, and administer medications as needed.

Responsibilities:

• Provide direct resident care, including vital signs, bath, dress, and grooming

• Administer medications and treatments as prescribed by physicians

• Monitor resident conditions and report any changes or concerns to the nursing staff

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Collaborate with the care team to ensure seamless transitions and quality care

• Maintain accurate and up-to-date resident records

• Participate in quality improvement initiatives to ensure high standards of care

Requirements:

• Current LPN license in the state of Ohio

• 0 years of experience (new graduates welcome to apply!)

• Ability to work a night shift schedule (variable hours)

• Strong communication and interpersonal skills

• Ability to lift, push, and pull patients and equipment
