About the Role
At Enlight Hospice, we believe every moment matters. We are looking for an empathetic, driven Hospice Social Worker (MSW) to be the dedicated champion, advocate, and main point of contact for our patients and their families during end-of-life care.
In this role, you won't just manage a caseload—you'll bring warmth, stability, and critical guidance when families need it most. Working side-by-side with our supportive Interdisciplinary Group (IDG), you will translate care plans into real-world comfort, dignity, and peace of mind.
What You’ll Do
- Be the Support System: Conduct initial psychosocial assessments (within 5 days of admission) to build custom, goal-driven plans of care covering emotional, social, financial, and environmental needs.
- Work Remotely & In the Field: Conduct meaningful, face-to-face visits with patients in their homes, then complete your charting and follow-ups remotely within your guaranteed weekly hours.
- Collaborate & Consult: Partner closely with our IDG and QAPI teams, serving as the go-to expert on how social and emotional factors impact each patient’s overall health.
- Provide Continuous Care: Monitor evolving emotional needs, evaluate care effectiveness, and adjust support as patients transition—providing intensive visits during critical phases and the final days of life.
- Connect the Dots: Guide families through community resources, assist with discharge planning when needed, and coordinate visit schedules directly with Case Managers.
- Adapt & Lead: Support the broader IDG team even when families decline direct social work services, and step in as Volunteer Coordinator for your caseload when required.
What You Bring
- Education: Master of Social Work (MSW) from a CSWE-accredited program.
- Experience: Hospice, palliative care, or home health experience is a huge plus!
- Mindset: An open heart and mind—capable of embracing diverse cultures, beliefs, and lifestyles with respect and authenticity.
- Skills: Clear communication, strong self-confidence, and a proactive, problem-solving attitude.
- Logistics: Valid driver’s license, an insured vehicle in good working order, and readiness to travel to patient homes across our service area.
Work Environment & Physical Expectations
- Field & Remote Split: Field travel to patient residences combined with remote home-office hours for charting and team coordination across your 12-16 hours guaranteed weekly hours.
- Physical Tasks: Patient home environments vary and may not always be wheelchair accessible. Physical expectations include walking, sitting, standing, stooping, and performing observational checks.
- Safety & Tech: Standard safety protocols apply, including awareness of bloodborne pathogens. Familiarity with standard medical technology, computers, and phone systems is required.
