Location: Eureka, California – Northern California coastal region

\n

Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30), every other weekend required. Caseload up to 25 patients. 13-week assignment with 40 hours of orientation.

Requirements:\n

  • Active California RN license or Compact license (must be in hand at submission)
  • \n
  • Minimum 2 years of acute care hospital case management experience required (must be current within the last year)
  • \n
  • Epic charting system experience required
  • \n
  • Experience with HMOs, IPAs, advance directives, POLST, and protective services processes
  • \n
  • Knowledge of admission criteria, discharge planning, DRG, appeals and denials, and care coordination
  • \n
  • Familiarity with HIPAA guidelines, CMS regulations, and Joint Commission standards
  • \n
  • Two professional references covering 1 year within the last 3 years
  • \n
  • Commitment to work 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
