Job Summary\nCuready Healthcare Staffing is seeking a qualified RN, Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.
Responsibilities\n- \n
- Coordinate patient-centered care across interdisciplinary teams to ensure optimal outcomes. \n
- Manage patient discharge processes, ensuring adequate post-care instructions and resources. \n
- Facilitate communication between care teams, patients, and families regarding treatment plans. \n
- Utilize Epic for maintaining accurate and up-to-date patient records. \n
- Perform regular needs assessments and adjust care plans accordingly. \n
- Work every other weekend as required, adhering to the facility's staffing policies. \n

- Current RN license with active, unrestricted status. \n
- Minimum of 2 years of acute care experience in a hospital setting. \n
- Experience with case management and care coordination. \n
- Proficient in Epic or similar electronic health records systems. \n
- First-time travelers are accepted with clinical approval. \n
- Flexibility to float between departments as needed based on staffing requirements. \n
