Responsibilities:
\n- \n
- Assess home health patients in stable and emergent situations to identify physical, psychosocial, and environmental needs. \n
- Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care. \n
- Obtain data on psychological, social, spiritual, and cultural factors affecting patient/family health and incorporate into the POC. \n
- Maintain accurate, up-to-date patient records, documenting visits, contacts, problems, plans, and changes. \n
- Provide care while following infection control measures to protect staff and patients. \n
- Educate patients and families on care, medication, diet, and safety, utilizing appropriate teaching methods. \n
- Supervise and provide clinical directions to HHA/STNA/LVN to ensure quality service. \n
- Assess and reassess pain, utilizing appropriate management techniques, and educate about pain control. \n
- Arrange referrals to appropriate agencies when necessary. \n
- Monitor cases for compliance with third-party payer requirements. \n
- Perform on-call responsibilities and provide services to patients/families as assigned. \n
- Communicate effectively with patients, families, team members, and healthcare professionals. \n
- Attend interdisciplinary team meetings as needed. \n
- Perform additional duties as assigned or requested. \n
