Requirements:

- 2+ years of experience as a PT \n
- Qualified applicants MUST have at least 1 year of experience in Home Health \n
- Qualified applicants MUST have an active Physical Therapist license \n
- Be willing to obtain Florida or compact licensure \n

Responsibilities:
\nAs a healthcare professional in a Home Health setting, you will:

- Deliver one-on-one, high-touch care to patients in their homes, including skilled assessments and care plan creation (PT). \n
- Collaborate with families and interdisciplinary teams to optimize patient outcomes, utilizing your expertise in functional mobility and rehabilitation. \n
- Perform evaluations using industry-standard procedures, ensuring compliance with state and federal regulations. \n
