American Traveler is seeking a Home Health Physical Therapist with at least 1 year of home health experience and proficiency in OASIS documentation for a 13-week travel assignment.

- Home health setting with patient visits conducted in residential environments \n
- Caseload expectation of 6 points per day or 30 points per week \n
- Day shift schedule, Monday–Friday, 8:00 AM–4:30 PM (5x8-hour shifts) \n
- Patient population includes geriatric, orthopedic, neurological, and cardiopulmonary diagnoses such as total hip/knee replacement, post-stroke rehab, CHF, COPD, and dementia \n
- Floating may be requested based on facility needs \n

- Minimum 1 year of recent home health PT experience required \n
- Proficiency in OASIS documentation required \n
- Current BLS certification required (must be through the American Heart Association) \n
- Active PT license required if applicable \n
- Strong skills in gait training, therapeutic exercise, functional mobility, and patient assessment \n
- Experience with standard home health equipment including walkers, gait belts, crutches, and assistive devices \n
- SSN and a copy of a valid driver's license required for consideration \n
- One supervisory reference required for consideration \n
- Prior permanent employees must have been separated from the facility for at least 6 months before being considered \n

- Responsibilities include evaluation, plan of care development, OASIS completion, reassessments, and discharge documentation \n
- Business casual attire or solid color scrubs are acceptable \n
- Local candidates are welcome and accepted at the same rate \n
- RTO should be kept to a minimum — no full calendar weeks off preferred, and no more than 7 days total during the assignment; all RTO must be submitted upfront and no additional RTO will be approved after an offer is made \n
- Candidates may not simultaneously hold a travel assignment and a per diem contract with the same health system; one must be chosen \n
