• Develop and implement individualized care plans in collaboration with the healthcare team

• Communicate effectively with residents, families, and healthcare team members to ensure seamless care coordination

• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

• Assume responsibility for specific patient care areas, including vital sign monitoring, medication management, and documentation
