Speech-Language Pathologist (SLP) – Full Time & Per Diem
Golden Crest Subacute Rehabilitation Center
Location: 100 Smithfield Road, North Providence, RI 02904
Golden Crest is seeking a compassionate and dedicated Speech-Language Pathologist (SLP) to join our rehabilitation team in an in-house subacute rehabilitation setting. We are currently accepting applications for Full-Time and Per Diem opportunities.
The Speech-Language Pathologist will provide skilled speech, language, cognitive, and swallowing therapy services to residents, helping them improve communication abilities, enhance safety, and maximize their independence and quality of life.
Responsibilities:
- Evaluate residents and develop individualized speech-language therapy treatment plans
- Provide skilled therapy services addressing speech, language, cognition, communication, and swallowing disorders
- Conduct swallowing evaluations and provide appropriate interventions as needed
- Monitor resident progress and adjust treatment plans in collaboration with the interdisciplinary team
- Document evaluations, daily treatments, progress notes, and discharge summaries accurately and timely
- Educate residents, families, and caregivers regarding therapy goals and strategies
- Collaborate with nursing staff, physicians, and other rehabilitation professionals to support comprehensive resident care
Qualifications:
- Current Speech-Language Pathologist license in the state of Rhode Island
- Master’s degree from an accredited Speech-Language Pathology program
- Certificate of Clinical Competence (CCC-SLP) preferred
- Experience in skilled nursing, subacute rehabilitation, or similar healthcare settings preferred
- Strong clinical, communication, and organizational skills
- Commitment to providing compassionate, resident-centered care
