Job Summary:

The MAT Program Specialist provides social work, counseling, and case management services to individuals receiving Medication-Assisted Treatment (MAT) services. This role supports reentrants transitioning from correctional facilities into the community by coordinating treatment referrals, tracking services, conducting assessments, and ensuring continuity of care. The position works collaboratively with SCI staff, parole staff, treatment providers, and multidisciplinary teams to support individuals’ recovery and social functioning.

Key Responsibilities:

  • Coordinate with SCI MAT Program Specialists regarding reentrants returning to facilities for detox and MAT eligibility assessments.
  • Assist with MAT referrals, including scheduling medication appointments and treatment services in the community.
  • Ensure assigned individuals receive required benefits, services, and treatment support according to care plans.
  • Maintain accurate tracking records and update MAT release databases/spreadsheets.
  • Attend weekly Vivitrol conference calls and additional program meetings as required.
  • Document all client interactions and services in systems such as Captor and Sapphire.
  • Complete required trainings and maintain compliance with DOC policies and procedures.
  • Meet with reentrants with positive urine drug screens to assess needs and provide treatment referrals.
  • Complete GPRA assessments, including initial, six-month, and discharge evaluations.
  • Provide services at SCI facilities, BCC locations, parole offices, and treatment provider sites as needed.
  • Communicate treatment plans and updates with correctional staff, parole staff, and treatment teams.
  • Follow MAT/AOD treatment frameworks, policies, and procedures established by the DOC.
  • Provide counseling, social work, and case management services to improve social, economic, and physical well-being.
  • Assess psychosocial strengths, challenges, service needs, and eligibility for available resources.
  • Counsel individuals using appropriate treatment approaches and interventions.
  • Participate in multidisciplinary meetings to review assessments, family history, and treatment progress.

Required Qualifications:

  • Bachelor’s degree in Social Work, Human Services, or related field required.
  • Knowledge of professional social work principles, practices, and methods.
  • Knowledge of case management principles and counseling techniques.
  • Understanding of individual and group behavior and psychosocial needs.
  • Knowledge of mental health, substance use, rehabilitation, and social service resources.
  • Understanding of HIPAA requirements and state confidentiality regulations.
  • Ability to interpret policies, procedures, and program guidelines.
  • Proficiency in Microsoft Excel and database documentation.
  • Strong written and verbal communication skills.
  • Ability to build and maintain effective professional relationships.

Preferred Skills:

  • Experience supporting individuals with substance use disorders.
  • Experience in social work, counseling, or case management settings.
  • Knowledge of MAT/AOD treatment programs and recovery services.
  • Experience working with correctional, parole, or community-based populations.
