Location: ClewistonEmployment Type: Part-Time 1099Schedule:  2-3 days a week Job SummaryThe Post-Acute Nurse Practitioner (APRN) provides comprehensive medical care to patients in skilled nursing facilities. This role is responsible for facility rounds, patient assessments, diagnosis and treatment of medical conditions, care coordination, and follow-up management. The APRN collaborates closely with physicians and interdisciplinary teams to ensure high-quality post-acute patient care.Primary ResponsibilitiesExamine patients and obtain comprehensive medical historiesPerform routine facility rounds and patient assessmentsAdhere to proper charting and documentation protocolsCollaborate with physicians and healthcare teams to develop and implement care plansOrder and interpret diagnostic tests and imagingAnalyze test results, diagnose conditions, and communicate findings to patients and staffPrescribe medications and administer treatments as appropriateSubmit patient care plans and goals for periodic physician reviewPromote preventive care and provide patient education on disease management, hygiene, and nutritionAdminister immunizations and vaccinations as requiredRefer patients to specialists when necessaryReport deaths and contagious diseases to appropriate governmental agenciesPerform other duties as assignedEducation & ExperienceCurrent Florida APRN license in good standingCurrent National Board CertificationActive DEA registrationMinimum one (1) year post-acute or skilled nursing experience preferredKnowledge, Skills & AbilitiesExcellent written, verbal, and electronic communication skillsStrong initiative, creativity, and problem-solving abilityAbility to work effectively within interdisciplinary healthcare teamsCommitment to delivering high-quality patient-centered careAdditional QualificationsPrior experience with Electronic Medical Record (EMR) systemsBilingual (English/Spanish) preferredBenefitsMalpractice insurance including tail coverageReporting RelationshipReports to the Supervisory Physician and Post-Acute Executive DirectorPhysical RequirementsAbility to communicate effectively with patients and staffAbility to sit or stand for extended periodsAbility to use computers, printers, and scannersAbility to perform repetitive hand and wrist movementsAbility to reach with hands and arms to handle or feel objects
