Responsibilities

• Perform evaluations and establish treatment goals
• Develop individualized treatment plans
• Provide skilled physical therapy services
• Complete clinical documentation
• Educate caregivers on home exercises
• Coordinate care with physicians and therapy team members
• Monitor patient progress and adjust treatment as appropriate

Requirements

Arizona PT or DPT license, CPR certification, AHCCCS ID, Article 9 certification, reliable transportation, and strong clinical reasoning skills.
