You'll communicate with patients in person, over the phone, and electronically to identify needs and then work with your team to implement the appropriate plan of care. Attention to detail will be vital as you document care in clinical systems, support care gap closure, and assist with other responsibilities as needed while complying with all healthcare regulations and CHP procedures. LPN - GPN QUALIFICATIONSYou will need at least one of the following: Licensed Practical Nurse certification from an accredited college or technical school, or a written acknowledgement of Graduate Practical Nurse status from the Florida Board of NursingAbility to read, understand, and follow safety rules, maintenance instructions, and procedure manualsAbility to write clear routine reports and correspondenceAbility to clearly present and explain information one-on-one and in small groupsAdaptable problem-solving skillsCollaborative team spiritMath skills (adding, subtracting, multiplication, division, fractions, decimals, ratios, etc.) with the ability to create and interpret bar graphsTechnology proficiency with the ability to learn and use Windows-based software1 year of related experience - preferred, not requiredWOULD YOU LIKE TO JOIN US?We are a not-for-profit health care organization that has served the panhandle since 1982, delivering high-quality, affordable, and patient-centered care.
