Key Responsibilities

  • Perform comprehensive patient assessments and ongoing evaluations
  • Develop, implement, and revise individualized care plans
  • Administer medications, injections, and IV therapies as prescribed
  • Provide wound care and monitor healing progress
  • Monitor and manage chronic conditions (e.g., diabetes, heart failure, COPD)
  • Educate patients and family members on disease management, medications, and safety
  • Document all patient care accurately in electronic health records (EHR)
  • Communicate changes in patient condition to physicians and care teams
  • Coordinate care with physical therapists, occupational therapists, social workers, and other healthcare professionals
  • Ensure compliance with agency policies, state regulations, and federal guidelines
  • Participate in case conferences and quality improvement activities

 

Required Qualifications

  • Active Registered Nurse (RN) license in the state of practice
  • Graduate of an accredited nursing program
  • Current BLS/CPR certification
  • Minimum of 1–2 years of clinical nursing experience (home health experience preferred)
  • Valid driver’s license and reliable transportation
  • Ability to work independently and manage time effectively

 

Preferred Qualifications

  • Experience in home health, hospice, geriatrics, or medical-surgical nursing
  • Strong wound care and IV therapy skills
  • Knowledge of Medicare/Medicaid regulations
  • OASIS documentation experience
