Job Details:

• Title: Social Worker (LBSW)
• Location: Salina, KS
• Pay: Starting at $24.77/hour (based on education & experience)
• Relocation Assistance: Up to $5,000
• Schedule: Full-Time | Day Shift | 8-hour shifts | Possible on-call | Weekend requirements discussed during the interview

We are looking for a dedicated Licensed Bachelor Social Worker (LBSW) to provide case management services, coordinate patient care, and connect patients and families with the resources they need to achieve positive health outcomes.

-> Responsibilities

• Assess patients' psychosocial needs and develop individualized care plans.
• Coordinate discharge planning and community resource referrals.
• Collaborate with physicians, nurses, and interdisciplinary teams.
• Advocate for patients and families while ensuring continuity of care.
• Monitor patient progress and document assessments and interventions.
• Promote quality, patient-centered, and efficient care.

-> Qualifications

• Bachelor's Degree in Social Work or related social services field (Required)
• Current Kansas LBSW License (Required)
• Two years of hospital-based social work experience preferred
