Responsibilities:

• Provide compassionate and individualized social work services to residents, family members, and caregivers

• Conduct comprehensive initial assessments and develop personalized care plans that address the physical, emotional, and social needs of our residents

• Collaborate with interdisciplinary teams to ensure seamless care coordination and high-quality care delivery

• Facilitate resident and family counseling, education, and support to promote independence, self-advocacy, and overall well-being

• Develop and lead groups, workshops, and programs that foster social connections, community building, and personal growth

• Maintain accurate and detailed records, including assessments, care plans, and progress notes

• Stay up-to-date with industry developments, best practices, and regulatory requirements to ensure compliance and excellence in care delivery

Requirements:

• Licensure: Current and valid LSW or LCSW licensure in the state of Michigan

• A strong commitment to delivering compassionate, patient-centered care with a focus on holistic well-being

• Excellent communication, interpersonal, and organizational skills

• Ability to work effectively in a fast-paced environment with multiple priorities and deadlines

• Strong attention to detail and ability to maintain accurate records
