Job Description:\n\nReports to: Clinical Assistant Program Director\nMonday- Friday 8:00am-4:30pm, Weekends on call rotation\nOn-Site\n\nPosition Summary\nWorking in accordance with the established Everstand Policies and Procedures, federal and state regulations as well as the applicable State Board(s) of Nursing to provide administrative and clinical healthcare services as well as direct nursing services to youth placed within the program. The Caminos Medical Coordinator is an active member of program leadership responsible for care coordination, oversight and administration of medications; coordination of routine and speciality medical and dental care utilizing a network of community providers; health education for youth and staff; and all administrative tasks.\n\nWhy Join Everstand?\nFind joy and purpose in your career. Make it happen.” \nProvide oversight and coordination of medical care for the Caminos shelter program, including the delivery of high-quality services in accordance with regulatory timeframes to ensure that youth are provided with safe and supportive programming.\nEnsures all Caminos Program residents receive quality and timely medical and dental care in accordance with state and federal regulations.\nResponsible for developing, maintaining and overseeing working relationships with community providers to ensure all medical appointments and follow-up recommendations are documented, monitored and reported according to state and federal standards.\nArrange and communicate with staff for all in house and outpatient medical appointments/services to include hospital emergencies and/or admissions, medical appointments, laboratory services, vaccine clinics, x-rays, and ultrasounds.\nSchedule appointments, accompany youth as necessary on specialty medical appointments, and communicate provider’s questions/concerns to ensure accuracy and patient safety.\nProduce a daily resident appointment list and ensure all staff are aware via email and hard copies in staff mailboxes.\nAudit, track, and schedule required physical, vision, and dental appointments.\nResponse for the initial training and renewal process for non-medical staff to be certified by the State in medication administration, including completion of purchase orders.\nOrganize and maintain resident medical charts, physical and electronic, utilizing program required agency and federal databases.\nUpdate and file patient medical records, including retrieval and copies of medical records necessary for staff reports and outpatient services.\nAssures completeness of documentation for the clients and for associated nursing functions such as the MAR, nursing screening exams, PPD screens, vaccinations, vital signs, laboratory logs, peer reviews and other quality initiatives, nursing sick call screens, consultations and referrals, in service training, and other activities of the healthcare suite.\nEnsures completeness of documentation of medical services provided to clients as required in federal and state databases, including the completion and submission of medical serious incident reports.\nResponsible for timely submission of Treatment Authorization Requests, or other forms required for reporting and billing purposes.\nInventory and maintenance of all administrative and medical supplies and medications.\nMaintains regular and consistent communication with Caminos Medical Services Administrator and the designated ORR Medical Coordinator, including participation in regular Everstand and ORR sponsored training and guidance updates.\nPerforms administrative duties as requested by supervisor, including researching and updating protocols/procedures, obtaining educational materials, coordinating care at outlying facilities, tracking billings and expenditures, organizing in service training, assuring regulatory compliance of the health suite.\nCollaborate with other departments for various service needs, including but not limited to:\n\nEnsuring comprehensive care coordination.\nEnsuring access to mental and behavioral health services.\nProviding information, psychosocial education and local community resources as needed to identified sponsor placements.\nWork in partnership with the ESS Supervisor to schedule medical transportation\n\nDemonstrates an understanding of cultural differences and perspectives, including the ability to develop systems and practices that are inclusive of all staff members and responsive to the cultural needs of the client population served.\nEvaluates productivity of the medical department and develops plans to improve efficiency.\nOversees patient and staff satisfaction through excellent customer service, communication, teamwork, safe practices, attendance, punctuality, and strong leadership.\nMakes sound employment practices by adhering to all Everstand policies, procedures, and standards, local, federal and state regulations and employment laws.\nDemonstrates knowledge of and familiarity with both Everstand and Office of Refugee Resettlement Policy and Procedures relating to the care and supervision of Unaccompanied Children. This includes knowledge of legal background and authority governing the practice as related to working with Unaccompanied Children.\nDemonstrate a willingness and ability to learn and evolve with changes in the organization and in health care.\nObtain and maintain certification as a “user” in Everstand’s electronic health record software system, Cx360. \n\n \nClinical Duties\n\nCarries out physician orders and provides day-to-day nursing care as needed, including administering medications and vaccinations, simple lab tests, phlebotomy, throat culture, support to pediatrician, client education, interpretation of medical information, accompanying clients to surgical or extensive medical/emergency procedures, nursing intake, first aid, and emergency/sick call triage.\nResponsible for preparation of treatment room(s) for patients.\nAssist the providers and/or nurses during exams with drawing labs, changing dressing, removal of sutures, etc.\nExplain treatment procedures to patients and assist with education.\nInstruct patients about medications and special diets.\nMedication administration implementing the six (6) Rights of Medication Administration.\nMedication administration with basic knowledge of medications and their use, drug reactions, and methods of administration.\nMedication administration consisting of: observing the youth for changes in physical conditions and/or behaviors, reporting these observations as soon as possible, receiving the prescribed medication, storing the medication safely, ensuring medications are taken, recording information promptly and correctly on appropriate forms.\nReport medical emergencies immediately to designated personnel.\nMaintain confidentiality of all pertinent health information to assure youths’ rights are protected in adherence to HIPAA of Privacy and Security.\nTakes vital signs and/or observes patients to collect information and prepares report or notifies Registered Nurse.\nKeep work area(s) free of hazardous conditions to assure that all established safety rules and regulations are followed at all time in adherence with Infection Control Standards.\nAdvocate on behalf of the youths’ best interest and collaborate with their treatment team to ensure their health and safety.\nFacilitates training, education and assessment of skills, along with maintaining requirements for medication administration personnel as necessary.\nComplete CDC and OSHA training for hazardous waste, TB management, and blood borne pathogen standards.\nMust be willing to perform tasks that may involve exposure to the patients’ blood and/or body fluids.\n\n \nNon-Essential Functions \n\nServe as Nurse on call on a rotating schedule.\n\nCompetencies\n\nAccountability for Others\nAttention to Detail\nEmotional Control\nFlexibility\nFollowing Directions\nFrequent Interaction with Others\nHandling Stress\nMeeting Standards\nMonitoring Others\nOrganized Workplace\nPersonal Accountability\nRespect for Policies\nSensitivity of Others\nUrgency\nTime Management\n\nSupervisory Responsibility\n\nProvides direct supervision to the Caminos Medical Assistant, Program Nurse and Weekend nurses which includes regular individual guidance meetings and performance reviews, in accordance with agency policies and procedures.\n\nWork Environment\n\nThis job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.\n \nIn the nature of being at a residential and school living environment there may be times you experience conditions in which challenging behavior can be exhibited from our youth to include but not limited to, physical agitation, physical violence and/or emotion outburst.\n \nPhysical Demands\n\nThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Days and hours of work are Monday through Friday, 8:00 a.m. to 4:30 p.m. Eligible for flex schedule. Not essential for inclement weather.\n\nTravel\n\nThis position is responsible for WV Caminos programs and will need to travel as needed for training and program oversight activities. Travel is primarily local during the business day, although some out-of-area travel may be expected.\n\nRequired Education and Experience\n\nAssociate of Science Degree in Nursing\nPossession of an active compact Registered Nurse license without disciplinary infractions by the applicable State Board of Nursing, or the ability to obtain a compact license or be licensed in dual states (WV and PA) immediately upon hire.\nCertification to provide training, oversight and monitoring for direct care personnel who are AMAP certified, as per applicable State regulatory within the first six (6) months of hire. \nTwo (2) years of nursing experience required, preferably in a pediatric or group home setting\nBasic computer skills\n\n \nPreferred Education and Experience\n\nBachelors of Science Degree in Nursing\nExperience working with diverse populations, including refugee and/or unaccompanied children and adolescents\nBilingual English/Spanish\n\nAdditional Eligibility Qualifications\n\nAble to learn and effectively utilize different technology platforms.\nExcellent computer use knowledge with Microsoft Windows 10 and 365 Office products, including Word, Excel, PowerPoint, and Outlook.\nProven effective time management, planning, communication, and interpersonal skills\nAbility to problem solve.\nAbility to work independently.\nAbility to keep an electronic calendar (Outlook).\nDemonstrates knowledge and clinical expertise in nursing.\nAbility to communicate in a clear, concise manner both orally and written.\nExhibits excellent customer service, communication, teamwork, safety practices, attendance, punctuality, and strong leadership.\nAbility to understand medical abbreviations, terminology, and military time.\nBasic computer proficiency to include MS Word, Excel and Outlook.\nBasic mathematical skills, including conversions and metric system.\nKnowledge of symptoms and treatment for childhood diseases.\nKnowledge of normal and abnormal human behaviors.\nKnowledge of side effects of medication.\nMust exhibits ability to interact positively with a caring, nurturing and professional attitude.\nMust be flexible and able to multi-task under pressure and deadlines.\nAbility to plan, implement and track effectively.\nDemonstrate a willingness and ability to learn and evolve with changes in the organization and in healthcare.\nAbility to participate effectively on Interdisciplinary Treatment Teams.\nAbility to pass TCI certification both at employee orientation and per the schedule outlined by HR thereafter.\nAbility to pass CPR/First aid certification both at employee orientation and per the schedule outlined by HR thereafter.\nAbility to demonstrate the possession of a valid driver’s license prior to employment and ability to maintain the license and operate a motor vehicle thereafter.\nAbility to pass CPS/UII background check both prior to employment and per the schedule outlined by HR thereafter.\nAbility to pass a Tuberculosis Screening both prior to employment and per the schedule outlined by HR thereafter.\nAbility to pass a work-readiness physical performed by a certified medical professional both prior to employment and per the schedule outlined by HR thereafter.\nAbility to pass a Federal and State Fingerprinting Background Check and applicable Suit
