Responsibilities

  • Plan Development: Designing and implementing Individualized Service Plans (ISPs) tailored to a client's specific needs.
  • Monitoring & Documentation: Tracking a client's progress, adjusting goals, and keeping detailed records.
  • Team Coordination: Acting as a bridge between direct care staff, medical professionals, and the client's family.

Requirements

  • Educational Background: A four-year degree in an accredited human services field or a related field of study.
  • Experience: Minimum of 12 months of hands-on experience assessing, tracking, and implementing active treatment or service plans for individuals with IDD.
