A growing Skilled Nursing & Rehabilitation facility in Worcester, MA is seeking a compassionate Licensed Physical Therapist (PT) to join its rehabilitation team.

This is an excellent opportunity for both experienced Physical Therapists and motivated new graduates looking to build their careers in a supportive, patient-centered environment. You'll work alongside an experienced interdisciplinary team with mentorship, flexible scheduling, and a strong focus on quality patient care.

What You'll Do

• Evaluate patients and develop individualized treatment plans.

• Provide therapeutic exercise, gait training, balance training, neuromuscular re-education, and manual therapy.

• Help patients improve mobility, strength, balance, and independence.

• Recommend and train patients on assistive devices and mobility equipment.

• Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and rehabilitation professionals.

• Complete timely documentation using an electronic medical record system.

• Short-Term Rehabilitation

• Long-Term Care

• Memory Care

Common diagnoses include:

• Falls

• Fractures

• General medical conditions

• Post-acute rehabilitation

Qualifications

Required

• Graduate of an accredited Physical Therapy program (DPT or MPT)

• Active Massachusetts Physical Therapist License

• Excellent communication and patient care skills

Preferred

• Skilled Nursing Facility (SNF) or Long-Term Care experience

• NetHealth EMR experience

• Outpatient experience

Schedule

• Full-Time

• Monday-Friday

• Flexible scheduling available

• No on-call responsibilities

• Weekend coverage available for overtime (not required)
