INTEGRIS Health Hospice Physician – Full-Time in Metro OKC

\n

INTEGRIS Health is seeking a compassionate and collaborative Hospice physician to support Metro Oklahoma City Hospice Services.  The successful candidate will join our engaged interdisciplinary team and exhibit expertise in the provision of high-quality hospice care.

\n

Responsibilities include:

\n

\n
• Physician oversight for Medicare and Medicaid hospice programs, ensuring that medical care is provided in accordance with federal Conditions of Participation (CoPs) and quality standards
\n
• Provide hospice and palliative care consultations for inpatient and home hospice settings
\n
• Engagement with Quality metrics including Hospice Determinations, Diagnosis Relatedness, CTI completions, KPIs
\n
• Lead discussions regarding goals of care, advance care planning and symptom management
\n
• Inpatient/outpatient rounding and oversight, medication orders, admissions, IDG coverage weekly, and call coverage (added call compensation)Participate in care transitions and coordination across the continuum of care
\n
• Collaborate closely with interdisciplinary teams including nursing, social work, chaplaincy, pharmacy and case management
\n
• Shared call coverage with supportive physician team
\n
• Rotations at Hospice House in NW OKC – 12 bed unit (shared among several physicians)
\n

Qualifications:

\n

\n
• Oklahoma MD or DO license
\n
• BC/BE in Internal Medicine, Family Medicine, Geriatrics, or related specialty
\n
• Fellowship in Hospice & Palliative Care preferred though not required
\n
