Location

Position: Registered Nurse – Hospice

Location: South Bend, IN
Schedule: Full-Time | Monday–Friday | 9:00 AM – 5:00 PM
Setting: Community-Based Hospice (On-Site/Field Work)

As a Hospice RN, you will:

Provide compassionate, patient-centered hospice nursing care in homes, facilities, or community settings
Perform assessments, develop individualized care plans, and monitor patient progress
Educate families on care needs, comfort measures, and available support
Collaborate closely with the interdisciplinary hospice team
Provide emotional support to patients and loved ones with warmth and respect

Candidate Requirements:
- Active RN license (Indiana)
- Minimum 2 years of hospice experience
- Reliable transportation for field visits
