Description

Provide patients and their families with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services focus on social, emotional, and financial needs, including clinical assessment, crisis intervention, and collaboration with healthcare teams. Assist family members to support the client, facilitating appropriate discharge needs. This is a day shift position with an average case load of 15, requiring weekend rotation every 6th weekend.
