DIRECT SUPPORT PROFESSIONALS

About the Role:
Extol Haven Homecare LLC is looking for compassionate and dedicated Direct Support Professionals to join our team in Mentor, OH! In this rewarding role, you'll make a meaningful difference in the lives of individuals by providing personalized care and support that empowers them to thrive in their daily lives.

Responsibilities:

• Provide direct care and assistance to individuals with developmental, physical, or intellectual disabilities
• Support clients with activities of daily living (ADLs) including personal hygiene, meal preparation, and mobility
• Implement individualized support plans and document progress accurately
• Assist clients with community integration, social activities, and recreational outings
• Administer medications and monitor health conditions as directed
• Communicate effectively with supervisors, family members, and healthcare professionals
• Maintain a safe, clean, and supportive home environment for clients

Requirements:

• High school diploma or GED equivalent required
• Prior experience in direct support, caregiving, or human services preferred
• Valid driver's license and reliable transportation
• Ability to pass a background check and drug screening
• CPR and First Aid certification (or willingness to obtain)
• Strong interpersonal skills with a patient, empathetic, and person-centered approach
• Ability to work flexible hours including evenings, weekends, or overnight shifts
