Job Summary

We are hiring a Licensed Bachelor Social Worker (LBSW) to assist patients with discharge planning, care coordination, resource referrals, and psychosocial support while working closely with the healthcare team.

Requirements

• Bachelor's Degree in Social Work or related Social Services field
• Active Kansas LBSW License
• Hospital-based social work experience preferred
• Excellent communication and case management skills
