Lifespark – RN Case Manager, Home Health
Salary & Schedule
Salary: $88,000–$90,000 annually (DOE)
Schedule: Full-Time, Monday–Friday, 8:00 AM–5:00 PM, with every 6th weekend rotation and 2 holidays per year
Mileage Reimbursement: $0.70/mile
Position Summary
The RN Case Manager plans, organizes, and directs home health services, including Start of Care visits, care plan development, case management, caregiver supervision, documentation, and collaboration with physicians and the care team.

Key Responsibilities
- Complete assessments and Start of Care visits
- Develop and update care plans
- Provide skilled nursing care
- Coordinate with physicians and interdisciplinary teams
- Educate patients and families
- Complete documentation by 10:00 AM the next day
- Supervise Home Health Aides every two weeks
- Coordinate DME and supplies
- Manage travel and mileage
- Participate in orientation, safety, and annual education
Qualifications
- 2+ years RN experience (Home Health/Hospice preferred)
- Medicare Skilled Nursing and OASIS experience strongly preferred
- Experience supervising caregivers preferred
- Graduate of an accredited nursing program
- Current Minnesota RN license
- Must pass MVR check
