A Faith-Based, Nonprofit, Short-Term Rehabilitation and Skilled Geriatric Care facility sponsored by the Carmelite Sisters for the Aged and Infirm, we've served the community since 1964. We are seeking to hire a Social Worker/Discharge Planner.
- $23 to $27 per hour based on circumstances including skills, qualifications, degrees and certifications, prior job experience, training, market data, and other relevant factors.
- Full-Time schedule.
Social Worker/Discharge Planner Qualifications:
- Bachelor's degree in Social Work or a related field required.
- MDS and Short-Term Discharge planning experience required.
- Minimum of one year of Social Work in a healthcare setting.
- Must have Skilled Nursing Facility (SNF)/Subacute experience.
- Knowledge of Point Click Care or other Electronic Medical Records (EMRs).
Social Worker/Discharge Planner Job Summary:
- Plan discharge and referring residents to appropriate services.
- Conduct interviews with residents and families to gather social histories.
- Provide solutions for social and environmental challenges, including financial assistance, discharge planning, and referring to community agencies.
- Complete all documentation in the electronic medical record - MDS.
- Update resident files/charts, Progress Notes, Plan of Care and Assessment.
- Participate in the Interdisciplinary Care Conference Meetings.
- Assist resident and/or family members with emotional and financial issues.
- Assist in communication with families.
