\n Home Health RN Case Manager (RNCM)
\n FT salary of $65,000.00 - $80,000.00 DOE
\n Previous home health, case management and oasis experience ideal.
\n Job Summary
\n BrightStar Care of Greater Austin is looking for a full-time Home Health RN Case Manager to join our growing team. We are seeking highly motivated and professional applicants with prior home health experience and a compassionate sense of urgency.
\n What You'll Do:
\n Coordinate RN Case Management of the Austin-based BrightStar Care agency
\n Ensure compliance and safety are adhered on a consistent basis
\n Maintain a growth trajectory and adhere to goals as indicated by Leadership
\n Provide on-going communications to Leadership and the Director of Nursing
\n Other duties as assigned
\n What We'll Love About You:
\n Minimum of 1 year supervisory experience in Home Health
\n Registered Nurse (RN) with unencumbered license in the state of Texas
\n Strong electronic documentation and follow-up skills required
\n Computer literacy and proficiency with Microsoft Office suite and EMR system
\n Strong working knowledge Medicare processes and compliance
\n Willing to be on an On-Call rotation and attend off-site meetings as required
