Continuum Therapy Partners is seeking a compassionate and dedicated Physical Therapist Assistant (PTA) to join our rehabilitation team at Jerseyville Nursing & Rehab Center in Jerseyville, Illinois.

This part-time Physical Therapist Assistant opportunity offers the chance to provide high-quality physical therapy services in a supportive, collaborative skilled nursing facility where your clinical expertise makes a meaningful impact every day.

As a PTA, you will work under the direction of a licensed Physical Therapist to provide individualized treatment interventions that help adult and geriatric patients improve mobility, strength, balance, endurance, and functional independence.

Responsibilities

• Provide physical therapy treatments under the direction and supervision of a licensed Physical Therapist.
• Implement individualized treatment plans designed to improve patient mobility, strength, balance, and independence.
• Assist patients with therapeutic exercises, functional activities, and mobility training.
• Monitor patient progress and communicate updates with the supervising Physical Therapist.
• Collaborate with Physical Therapists, Occupational Therapists, Speech-Language Pathologists, nursing staff, and caregivers.
• Complete accurate and timely documentation according to company, state, and federal guidelines.
• Educate patients and caregivers on mobility strategies, exercise programs, and safety techniques.
• Provide compassionate, patient-centered rehabilitation services.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current Illinois Physical Therapist Assistant (PTA) license required.
• Skilled nursing or rehabilitation experience preferred but not required.
• New graduates are encouraged to apply.
• Strong communication, organization, and teamwork skills.
