Continuum Therapy Partners is seeking a compassionate and dedicated Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team serving Lancaster Health Services in Lancaster, Wisconsin, and Mineral Point Health Services in Mineral Point, Wisconsin.

This Full-Time or Part-Time Occupational Therapy Assistant opportunity offers the chance to provide meaningful patient care in supportive, collaborative skilled nursing facilities where your clinical expertise makes a daily impact.

As a COTA, you will work alongside a licensed Occupational Therapist to provide skilled occupational therapy treatments that help adult and geriatric patients improve independence, safety, mobility, and quality of life.

Responsibilities

• Provide occupational therapy treatments under the direction and supervision of a licensed Occupational Therapist.
• Implement individualized treatment plans designed to improve patients’ functional abilities and independence.
• Assist patients with activities of daily living (ADLs), mobility, coordination, strength, cognition, and safety awareness.
• Monitor patient progress and communicate updates with the supervising Occupational Therapist.
• Collaborate with Occupational Therapists, nursing staff, physicians, caregivers, and interdisciplinary team members.
• Complete accurate and timely documentation according to company, state, and federal guidelines.
• Educate patients and caregivers on therapeutic techniques, adaptive equipment, and safety strategies.
• Provide compassionate, patient-centered occupational therapy services.

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program.
• Current Wisconsin Certified Occupational Therapy Assistant (COTA) license required.
• Skilled nursing or rehabilitation experience preferred but not required.
• New graduates are encouraged to apply.
• Strong communication, organization, and teamwork skills.
• Passion for helping patients achieve greater independence.
