This travel assignment is for a Licensed Practical Nurse with long-term care experience. Contracts are scheduled through the holiday season with a flexible end date and typically begin on a Tuesday or Thursday. The assignment requires NY LPN licensure and BLS, and it emphasizes geriatric and adult patient care under RN supervision.

Responsibilities
- Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse
- Collect data on residents’ physical and psychosocial status; communicate findings to the RN and Interdisciplinary Team
- Contribute to developing an age-appropriate, culturally relevant plan of care; involve resident/family and healthcare team; implement nursing interventions to achieve outcomes
- Monitor resident outcomes and suggest revisions to the plan of care; initiate referrals as needed to address all aspects of care
- Maintain current nursing knowledge and competencies; participate in staff orientation and continuing education; support a collaborative practice environment
- Demonstrate leadership in the professional practice setting and apply geriatric care principles for adult and elderly residents

Requirements
- Active NY LPN licensure
- BLS certification (AHA or RQI)
- Minimum 1 year of LPN experience
- Prior experience serving in a long-term care facility
- Ability to provide care for adult and geriatric residents
- Strong communication and teamwork skills; ability to work with the Interdisciplinary Team
- Willingness to participate in a schedule that includes rotating shifts and holiday coverage
