Deer Meadows Home Health & Support Services is seeking a dedicated and compassionate Part-Time Physical Therapist Assistant (PTA) to provide exceptional home health services to patients throughout the community. Position Responsibilities

• Provide skilled physical therapy treatments in patients' homes under the direction and supervision of a licensed Physical Therapist.

• Carry out individualized treatment plans designed to improve mobility, balance, strength, endurance, and functional independence.

• Educate patients and caregivers on home exercise programs, safety techniques, and fall prevention.

• Monitor patient progress and communicate changes to the supervising Physical Therapist.

• Complete accurate and timely clinical documentation using Homebase Home Care EMR.

• Collaborate with physicians, nurses, and the interdisciplinary care team to ensure high-quality patient care.

• Maintain compliance with agency policies and all applicable state and federal regulations.

Qualifications

• Current Physical Therapist Assistant (PTA) license in PA

• Graduate of an accredited Physical Therapist Assistant program.

• Home health experience preferred, but not required.

• Experience with Homebase Home Care EMR is a plus.

• Excellent communication, organization, and time management skills.

• Current CPR certification (or ability to obtain).

• Valid driver's license, reliable transportation, and current auto insurance.
