Responsibilities

• Complete client assessments and reassessments

• Assist with care planning and documentation

• Monitor client health and communicate changes

• Provide nursing services within the LPN scope of practice

• Support caregiver training as needed

• Communicate professionally with clients, families, caregivers, and office staff

Qualifications

• Current Ohio LPN license in good standing

• Current CPR certification

• Valid driver's license, reliable transportation, and auto insurance

• Strong communication and organizational skills

• Home care experience is preferred but not required
