Gardens of Mayfield Heights is seeking a compassionate and motivated PRN Certified Occupational Therapy Assistant (COTA) to join our dynamic rehabilitation team. This position offers the opportunity to work collaboratively with therapists, nursing staff, physicians, residents, and families to deliver high-quality rehabilitative care that promotes independence, functional improvement, and enhanced quality of life.
Responsibilities
Provide occupational therapy treatments under the direction and supervision of a licensed Occupational Therapist.
Implement individualized treatment programs designed to improve residents' functional abilities and independence with activities of daily living.
Monitor resident progress and communicate findings to the supervising Occupational Therapist.
Accurately document treatments, resident responses, progress notes, and other required clinical information in a timely manner.
Collaborate with interdisciplinary team members to support resident care goals and discharge planning.
Maintain compliance with facility policies, professional standards, state regulations, and federal guidelines.
Participate in care conferences and team meetings as appropriate.
Ensure a safe and therapeutic treatment environment for all residents.
Associate's degree from an accredited Occupational Therapy Assistant program required.
Current Occupational Therapy Assistant license in the State of Ohio in good standing.
CPR certification preferred.
Previous experience in skilled nursing, long-term care, rehabilitation, or post-acute settings preferred.
Experience with Net Health documentation software preferred.
Experience with PointClickCare (PCC) preferred.
Strong communication, organization, and interpersonal skills.
Ability to work effectively as part of an interdisciplinary team.
