This is a high-impact position responsible for building strong referral relationships with hospitals, physicians, case managers, discharge planners, and healthcare providers to increase admissions and drive census growth.

Position SummaryThe Hospital Liaison is responsible for developing and maintaining referral partnerships with hospitals and healthcare professionals while promoting the clinical capabilities and services of our skilled nursing facility. This role plays a critical part in achieving occupancy goals through strategic business development, exceptional customer service, and timely management of referrals.
Primary ResponsibilitiesDrive census growth by developing and strengthening referral relationships throughout the Greater Rockford market.
Build lasting partnerships with hospital case managers, social workers, discharge planners, physicians, and healthcare providers.
Conduct daily hospital visits and maintain a consistent presence within assigned accounts.
Respond promptly to referral opportunities and coordinate admissions with the interdisciplinary team.
Educate referral sources on the facility's clinical programs, rehabilitation services, and specialty care offerings.
Identify new business opportunities and implement strategies to increase market share.
Track referral activity, admissions, and marketing outcomes using CRM and reporting tools.
Represent the facility at healthcare networking events, community organizations, and professional meetings.
Collaborate closely with the Admissions, Nursing, Therapy, and Executive teams to ensure smooth patient transitions.
Monitor competitor activity and market trends to identify opportunities for growth.
Minimum of 2 years of healthcare sales, hospital liaison, admissions, or business development experience.
Experience with Skilled Nursing Facilities (SNF), post-acute care, rehabilitation, home health, hospice, or long-term care strongly preferred.
Existing relationships within hospitals and healthcare systems in the Greater Rockford area are highly desirable.
Excellent communication, presentation, and relationship-building skills.
Strong organizational and time-management abilities.
Self-motivated with the ability to work independently while meeting census goals.
Proficient with Microsoft Office and CRM systems.
Reliable transportation for daily local travel.
Bachelor's degree in Healthcare Administration, Business, Marketing, or a related field (preferred but not required).
Knowledge of Medicare, managed care, and post-acute referral processes.
Understanding of hospital discharge planning and skilled nursing admissions.
