What We’re Looking For:

• Must have an active APRN license in CT

• Strong clinical judgment, interpersonal and communication skills

• Comfortable working independently in multiple SNFs and collaborating with facility teams

• Willingness to travel within CT to a full-time caseload of apx 8 facilities
