As a Social Worker, you will play a vital role in supporting our residents' social, emotional, and psychological well-being. You will assist in their adjustment to our facility, help develop comprehensive care plans, and work closely with families to ensure a smooth transition and discharge process.

• Assist residents in adjusting to the facility and navigating room changes.
• Provide emotional and social support to residents and families.
• Facilitate family group meetings and assist with personal needs.
• Collect social history data and assist in discharge planning.
• Complete and maintain documentation, including MDS and Care Plans.
• Review and educate residents and staff on residents’ rights.
• Assist families with engraving or marking personal belongings.
• Participate in process improvement initiatives to enhance resident care and workflow.
• Support special projects and perform additional duties as assigned.

• Primarily indoors within a long-term care facility, including resident rooms.
• Work among healthcare staff, residents, and guests.
• Potential exposure to:
• Slippery floors, chemicals, and hazardous materials.
• Bloodborne pathogens and communicable diseases.
• High-stress medical or life-threatening situations.
