Admissions Coordinator

We are seeking a motivated, relationship-driven Admissions Coordinator to join our team. This individual will play a key role in supporting census growth, managing referral relationships, and ensuring smooth transitions of care for residents entering our facilities. The ideal candidate will be organized, customer-service focused, and knowledgeable in skilled nursing and post-acute care admissions processes.

Responsibilities of Admissions Coordinator

  • Demonstrate a clear understanding of facility admissions criteria, census goals, payer mix, and clinical capabilities.
  • Monitor and understand actual vs. budgeted performance related to census and payer mix.
  • Identify primary and secondary referral sources for skilled nursing facilities and actively develop relationships to increase referrals and admissions.
  • Build collaborative working relationships with facility leadership teams and external referral partners.
  • Ensure seamless, customer-friendly transitions between hospitals, referral sources, and facilities.
  • Develop and maintain a routine outreach schedule to strengthen community presence and educate referral partners and potential residents on post-hospital care options.
  • Implement and oversee referral source satisfaction surveys to maintain strong communication and position facilities as preferred providers.
  • Participate in marketing and outreach initiatives as assigned.
  • Perform other duties as assigned.

Qualifications of Admissions Coordinator

  • Experience in admissions, healthcare marketing, case management, or long-term care preferred.
  • Strong communication and relationship-building skills.
  • Knowledge of insurance, payer sources, and post-acute care services preferred.
  • Ability to work collaboratively in a fast-paced healthcare environment.
  • Excellent organizational and customer service skills.
