Job Summary and Qualifications
Works within the Quality Department with a primary focus to abstract, interpret and present data for assigned databases or registries with the objective of assisting the organization in demonstrating excellence in health and clinical outcomes. This position requires a person with a positive attitude who is self-directed. Ability to competently use computer software systems for data collection and analysis is required.
Working as a professional registered nurse, this person will also assess appropriate inclusion/ exclusion of data based on defined data dictionaries.
Other pertinent duties are: attaining and maintaining proficiency in data abstraction and entry; generating routine and ad hoc reports as directed within required time frame.
Collects and maintains accurate (>90%) performance improvement data activities via selected outcomes software systems. A high degree of skill performing the activity is required as the performance data gathered for external public reporting if it is not accurate and not acted upon, the hospital reimbursement will be adversely affected.
This person will also screen medical records utilizing pre-determined medical staff, and regulatory health center criteria. Identifies medical staff variances from criteria and routes variances to the appropriate physician for review.
Participates as directed in PI teams to identify improvement opportunities.
Core Measures: Concurrently assembles a daily core measure patient list and notifies the clinical departments of their Core Measures patients.
Daily rounds on the identified Core Measure patients and assists with communication of care interventions needed with the patient’s healthcare team.
Abstracts the Core Measures data from the patient’s chart and enters the data into the COMET database.
Populates and analyzes data into reports used for communication on performance for the Core Measures indicators.
Coordinates data collection/abstraction/measurement, and analysis pertaining to regulatory and organizational performance requirements;
Provides education to help improve organizational performance
What qualifications you will need:
- Minimum 3 years hospital clinical experience clinical nursing experience; 3 years of recent acute care clinical nursing experience preferred. Proficiency in use of Microsoft Office, proficiency in the use of spreadsheets including pivot tables, graphing and basic statistics; knowledge of Quality Improvement methodology and team facilitation.
- Licensed as a registered professional nurse in the state(s) of practice and/or has an active compact license, in accordance with law and regulation within 90 days of graduation.
