Nocona Rehabilitation & Care Center is seeking a dedicated Per Diem Physical Therapist (PT) to join our compassionate in-house therapy team. If you're passionate about helping residents regain strength, mobility, and independence, we'd love to meet you!

What You'll Do

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans.

• Provide evidence-based therapy interventions to improve mobility, balance, strength, endurance, and overall function.

• Collaborate with physicians, nursing staff, and the interdisciplinary team to deliver exceptional resident care.

• Monitor resident progress and modify treatment plans as appropriate.

• Complete timely and accurate clinical documentation in accordance with regulatory and facility requirements.

• Educate residents and caregivers on therapeutic exercises, safety, and fall prevention strategies.

• Maintain the highest standards of patient care and professional practice.

Qualifications

• Current Texas Physical Therapist (PT) license in good standing.

• Graduate of an accredited Physical Therapy program.

• Skilled nursing or rehabilitation experience is preferred, but new graduates are encouraged to apply.

• Strong communication, interpersonal, and organizational skills.

• Ability to work independently and as part of a collaborative rehabilitation team.
