Compensation
Salary Range: $90,000 – $100,000 per year
Location: Midtown Manhattan, NY
Hybrid (3 days onsite / 2 days remote)
Position Summary
As a Transition of Care Nurse Care Manager, you will oversee and coordinate patient care as members transition between healthcare settings and back into the community. This role focuses on reducing care disruptions, preventing readmissions, and ensuring members have the resources, support, and education necessary for a successful recovery. Working collaboratively with interdisciplinary teams, you will play a vital role in improving patient outcomes and continuity of care.
Responsibilities
Schedule / Shift
- Shift details: Hybrid Schedule (3 days onsite / 2 days remote)
- Hours per week: 40
- Contract length (if applicable): Direct Hire
Key Duties:
- Coordinate discharge planning with inpatient clinical teams from admission through discharge to ensure safe transitions of care
- Collaborate with utilization management teams to review clinical information and develop appropriate discharge plans
- Conduct post-discharge outreach within 24 hours following hospitalization, ER visits, or skilled nursing stays
- Assist members with medication reconciliation, medication compliance, provider follow-up appointments, and access to necessary resources and equipment
- Document care management activities, discharge planning communications, and member progress accurately within electronic systems

Licenses & Certifications
- Required:
- Current, unrestricted New York State Registered Nurse (RN) license and registration
- Ability to obtain RN licensure in Connecticut, Pennsylvania, and New Jersey (company-sponsored)
- Additional certifications (if applicable): N/A
Skills:
- Strong knowledge of discharge planning, case management, and utilization review processes
- Excellent problem-solving, decision-making, and organizational abilities
- Strong communication and member advocacy skills
- Ability to work effectively in a fast-paced, evolving healthcare environment and collaborate across multidisciplinary teams
